Our Office Policies

Medication:

Refills:

Please bring a current list of all medications with you to your office visit. If you need a refill between visits, please ask your pharmacy to fax a request so we can quickly respond to your need. We kindly ask for a 48 hour notice if you are needing a refill.

Scheduled Medications:

(Nerve and Pain medicine) These types of medicines will not be prescribed on first visits. Our focus is primary care and overall wellness; therefore, our clinic does not offer long term pain management. We also do not prescribe any controlled medications as a first line treatment for anxiety and depression We can refer you to a specialist of your choice if you are in need of these services.

Appointments:

Tardiness:

Please arrive 10-15 minutes prior to your appointed time to go through the check-in process. If you are 15 or more minutes late for your appointment, we will reschedule you at a more convenient time, or you may choose to wait to be seen at a time that we can work you into the existing schedule in order to be fair to other patients.

No-shows:

Appointments will be marked as “no-show” if you do not keep your appointment. Please note that multiple no shows for appointments may result in a fee being assessed to your account.

Cancellations:

We require a 24 hour notice of cancellation. If you cancel your appointment within three hours of your appointment, it will be marked as a no-show because it is difficult for that same office visit to be opened up for another patient in that time-frame.

Payment:

All co-pays/deductibles are due at the time of your appointment. This is a contracted agreement between our office and your insurance company. We accept all major credit cards, checks and cash. Thank you.
